Mission Hospice accepts referrals 24 hours a day, 7 days a week, including evenings, weekends, and holidays.
Our team responds quickly, and in many cases, patients can be assessed the same day a referral is received, helping them access care without delays.
Mission Hospice is committed to providing a continuum of care for patients with life-limiting illnesses. We serve patients and families throughout the San Francisco Peninsula and South Bay with palliative and hospice care tailored to their changing needs.
Our interdisciplinary care may include skilled nursing, physician and nurse practitioner home visits, medical social work for emotional support and case management, personal care from hospice aides, therapies as directed by a hospice physician, and support from trained volunteers.
We work closely with your team to coordinate care and determine the most appropriate setting and level of support for each patient. Every care plan reflects the patient’s individual needs, with sensitivity to cultural, religious, and personal preferences.
Our dedicated staff and volunteers—including physicians who make home visits—help patients remain as comfortable as possible while providing families with the guidance and support they need.
We can also help your patients plan for their future care. We know these conversations can be hard – our team has the time, expertise, and tools to talk to your patients and help them complete their advance directives.
Patients may be referred by hospitals, physicians, caregivers, or family members. Our staff is also available for informational visits with individuals and families considering hospice or palliative care. These visits can take place in hospitals, nursing homes, assisted living communities, or private homes.
Our hospice house in Redwood City offers rooms for patients who need intensive, round-the-clock symptom management that cannot be provided at home, or who need caregiver respite. House placement depends upon acuity and availability. Patients typically stay in the hospice house a few weeks to a few months.
Our expert educators and clinicians also provide continuing education for healthcare providers, including training for nursing and social work students, hospice rotations for medical students, and in-services on a range of subjects for staff at care homes and assisted living facilities. Our Transitions program helps patients and families adapt to the changes that accompany living with a serious illness and offers supportive services to patients with life-limiting illness who are not yet physically or emotionally ready for hospice.
