The difficulty\, uncertainty\, and fear continue from diagnosis through the dying process\, at death\, and through grief. In this conversation hospice social worker Lisa and hospice nurse Gabby will share some of the questions they are most often asked and the answers they provide. By answering these questions\, they hope to ease some of the fears you might have. \nLisa and Gabby will also share their tools to comfort others on their journey. \nThis conversation will be helpful to anyone who has been given a terminal diagnosis or is providing care to someone who has. SUMMARY:*VIRTUAL* Take Charge! Guided Advance Care Planning (two-part workshop)
DESCRIPTION:What matters most to you? If you were facing serious illness\, what kind of care would you want? If you couldn’t speak for yourself\, who would? \nPlanning for your future care can bring peace of mind – to you and your loved ones. Advance care planning is a process that helps you decide and document what kind of care you would want – and what kind you would not want – if you have a health crisis and are not able to communicate or make decisions. \nIn this free 2-part online course (February 2 and 9)\, you will learn more about advance care planning and the steps you can take to help get the care you want\, based on what matters most to you. You will be guided through the process of thinking about what is important to you\, talking about it with those close to you (including your doctor)\, selecting someone to act as your representative\, and completing an Advance Health Care Directive to document your decisions. At the conclusion of this training\, you will have all the information you need to complete your Advance Directives and the tools to do it. \nInstructor: \nConnie Borden\, RN\, MSN\, ANP\, is an advance practice nurse with 28 years of experience in hospice and palliative care. She has led a local nonprofit hospice as executive director and worked on inpatient services as a palliative consultant. She had presented advance care planning information during her 15 years at San Francisco’s St. Mary’s Medical Center.
