Take charge! Advance Care Planning Tips
What matters most to you?
- If you were facing serious illness, what kind of care would you want?
- If you couldn’t speak for yourself, who would?
Taking charge of your future care can bring peace of mind — to you and your loved ones — and is especially important during the COVID-19 pandemic.
“Take charge! Advance Care Planning Tips” is a new filmed presentation of our popular public workshop, presented by Mary Matthiesen of Mission Hospice, Christina Irving of Family Caregiver Alliance, and Gwen Harris of Seniors At Home.
This will be the first public viewing of the film, followed by a panel discussion and Q&A. Materials to review in advance of the evening can be found online.
Whether you already have an advance health care directive, or don’t know what one is, bring your curiosity, your questions, and get ready to take charge of this area of your life.
This film was made available with generous support from Family Caregiver Alliance, the California Department of Health Care Services, and San Francisco Department of Disability and Aging Services.
To keep our community safe, our events are now being held online. Please register at least 60 minutes in advance to receive the login info needed to join the event.
Your Zoom link and Zoom phone numbers (for those calling in) will be in your email confirmation. THANK YOU!
