Advance Care Planning for Professionals

Staff are often required to speak about or support clients and families in planning for their future care (advance care planning). Yet where do we learn about how to start this difficult conversation, or steps we can take to begin? This course is designed for people who are new to or want a refresher course on the basics of advance care planning from the perspective of a client or family member.
This 2 hour session will give tips to help caregiving staff to:
- Understand what advance care planning is — and isn’t
- Identify 4 key steps to supporting an informed advance care planning process
- Explore what matters most using a tool that can support client conversations
- Learn about tools for starting the conversation and documenting client wishes and preferences
Participants will each receive 3 tools that can be used with clients and family members following the course.
