It’s about life! Mission Hospice & Home Care helps patients live life to the fullest, offering comfort and compassion for those with life-limiting illnesses. We are known for our personal, individualized palliative care – whether in patients’ homes, assisted living, board and care, or skilled nursing facilities. We see every day that our compassionate care gives patients and their loved ones the opportunity to cherish their time together.
Palliative care treats the whole person, addressing physical, emotional, and spiritual needs. Working as a team, specially trained nurses, doctors, social workers, and spiritual counselors work closely with patients and families to support quality of life and relieve the symptoms of disease. Palliative care can be given at any stage of a disease, and is offered across Mission Hospice & Home Care’s continuum of care.
We serve patients throughout the stages of a life-limiting illness, in three programs: Advanced Care (palliative home health care), Transitions, and Hospice. Our continuum of care ensures high quality, consistent support regardless of the program in which a patient is enrolled, and facilitates a smooth transition to hospice when the patient is ready. We are dedicated to helping people throughout the challenges of a terminal diagnosis, regardless of their medical coverage or ability to pay.
We tailor a care plan especially for each patient, with sensitivities to cultural, spiritual, and personal preferences. Our continuum of care allows patients (and, importantly, their families) to remain with the same support team throughout their journey.
