Hospice care…the way it works.

At Mission Hospice & Home Care, we provide physical, emotional, social and spiritual (all-inclusive) care for our patients and their families. Our emphasis is on quality of life once curative measures are no longer effective or desired. We are experts in pain control and symptom management.  We offer a unique team approach that utilizes professionals from several different medical disciplines who formulate a comprehensive, personalized plan of care for each patient. We work closely with our patients’ personal physicians, who always remains in charge of a patient’s care.

A family member usually serves as the primary caregiver and, when appropriate, helps make decisions for the hospice care recipient. Members of our staff make regular visits to assess the patient’s needs and provide additional care or other services. Mission Hospice & Home Care staff is available to answer calls 24 hours a day, seven days a week.

The Mission Hospice & Home Care team develops a care plan that meets each patient’s individual needs for pain management and symptom control, and the loved ones’ needs for support.

Our interdisciplinary team consists of:

• The patient’s personal Physician
• Hospice Medical Director
• Hospice Nurses
• Social Workers
• Home Health Aides
• Chaplain/Spiritual Counselor
• Bereavement Counselor
• Trained Volunteers
• Dietician
• Speech, Physical or Occupational Therapists and other Counselors, as needed

Together with family members, the hospice team manages the patient’s pain and symptoms, assists the patient and family with the emotional and spiritual aspects of dying, and provides needed medicines, medical supplies and equipment. 

The team may also:

• coach the family on how to care for the patient,
  
• deliver special services like speech or physical therapy
  
• make short term inpatient care available when pain or symptoms become too difficult to manage at home, or the caregiver needs respite time
  
• refers family to additional community resources such as in home support / attendant care
  
• provide bereavement care and counseling to surviving family and friends
