The Advanced Care Program

The Advanced Care Program (home care) provides skilled nursing care and medical social work services for people who are homebound with a life-limiting illness.

Advanced Care is prescribed by your doctor and is provided by a variety of health care professionals. It is covered by Medicare and private insurance.

Patients on the Advanced Care Program may pursue life-prolonging treatment options such as
chemotherapy and radiation when prescribed by their primary physician.

You may also require the services of an occupational therapist, physical therapist or speech therapist to help you transition from the hospital to a home environment.

Home health professionals may teach you or your caregivers how to care for wounds and manage medication. The goal is to maximize your physical, mental and social well being
within the context of your illness.
