What is the Transitions Program?
Our Transitions Program provides emotional support and practical assistance for patients with life-limiting illnesses. This free, volunteer supported, non-medical program complements the Advanced Care Program and allows us to serve the holistic needs of patients who are not yet ready or eligible for hospice care. It also enables us to provide a smooth and timely transition toward hospice, when appropriate.
