More about the Advanced Care Program

The Advanced Care Program includes the following components:

Skilled Nursing Care
We specialize in managing and providing education regarding all aspects of wound care, prevention, disease management and symptom control. We can help supervise your medications, as well provide I.V. therapy, colostomy and catheter care.

Medical Social Work
A serious illness or injury can affect many areas of your life. Our medical social workers can assist you with state programs like Medi-Cal and recommend local resources, including caregiver assistance and transportation. They also offer counseling and emotional support for you and your family.

Home Health Aide
Our home health aides provide hands-on help with the chores of daily living, such as hygiene, grooming, bathing, dressing, meal preparation and light housekeeping. They may also provide training and assistance for caregivers hired through an outside agency.

Physical Therapy
Our physical therapists work with you to increase your strength and mobility within the limits of your illness. This may include developing a home exercise program, teaching you balance and fall prevention, or ordering mobility equipment such as a walker and teaching you how to use it.

Occupational Therapy
Our occupational therapists help you regain as much independence as possible in a home environment. This may include teaching self-care skills, pacing and breathing techniques, and utilizing safety equipment.

Speech Therapy
Our speech therapists assist patients who have difficulty speaking, hearing or understanding others. They are trained in all aspects of cognition, and they can identify and treat problems with swallowing and oral motor control.
